Please refer to the local campaign timeline page for deadline dates pertaining to toy applications before completing this form.

If you have a request for toys for your agency, please provide the name of the agency, point of contact information, the number of children you are requesting toys for along with the age and gender. If you have any questions, feel free to call me at 507-200-0582 or email me at adrian.mn@toysfortots.org. Requests should be submitted no later than November 25th, 2024.

I agree to the following terms:

Toys donated by Toys for Tots will not be auctioned, sold or otherwise turned over for monetary donations.
Toys donated by Toys for Tots will not be taken out of state for distribution.

I understand any violation of the above agreement will result in disqualification from future donations.

We are seeking your help to make this program as successful as possible and fair to all children. Please do not inflate the number of children on your toy request form. We do our very best to make sure each child receives two toys, one large and one small. We ask you to submit a request to only one distribution center. Sorting requests made to multiple centers costs the program valuable time that could otherwise be spent on collecting and distributing toys.

I provide my consent to receive automated SMS updates regarding my application status, as well as Toys for Tots campaign messages sent by the local chapter and the national Toys for Tots headquarters. Standard message and data rates may apply
